Dear Valued Client, As you are already aware, SILO’s Pathway to Wellness Counseling Center has ceased operations. As a result, we are informing you of the process for obtaining a copy of your medical records, should you wish to do so. To request your records, please contact SILO's main line at 631-880-7929 and ask to be transferred to Yahira DeLeon at Extension 147. She will assist you in processing your request. During the call, you will be asked whether you would prefer to receive your records electronically or through postal mail. Please let us know your preferred delivery method, and we will ensure your records are sent accordingly. We would also like to assure you that your medical records are stored securely and in compliance with all relevant privacy and security regulations, including HIPAA.
